Please fill this application out as completely as possible.  Please make note of the instructions below for filling out this form.

If a question does not apply to you for some reason, then enter N/A.  Do not just leave the field blank.  We need to know that requested information is not applicable to your situation.  Apps with a lot of blank fields are assumed to be incomplete, and will be dealt with accordingly.

You must provide us with a valid email address, and phone number for us to process your application.  We will be calling and/or emailing you.  If your email or phone do not work, we will deny the application.  Please double check your entries.

The address you provide MUST be your physical address.  Do not supply us with PO Box or other similar address information.  The address is being used to verify your physical location, and to do our virtual home inspection via Google Earth.

Due to our web host discontinuing support for Front Page Extensions, we are going back to the old school method.  Please download the PDF version here, and either print it and fill it out, or use a form filler that works with PDF documents.  If you have printed the application, and filled it out long hand, you will need to scan it into your computer, in PDF format, after you complete it.  If you simply filled it out using a form filler, then make sure you save the edited copy.

We try to get to applications within 48 hours.  However, if receive your application on a Friday, it may be Monday or Tuesday before you hear from us.  Our weekends are spent running adoptions, intake, and assessments.  We often are on the road, far from a computer, and that necessitates a longer lag time on applications received across the weekend.
